Please fill out one form per person.
Everyone MUST pre-register, no registration or payment will be accepted on the day of the event.
a. If paying with PayPal for more than one person, please put the same confirmation # from your receipt on the form for each person covered in the payment.
b. In addition, please put a note on your PayPal payment with the your name and "KokaDamas Workshop".
Please bring all cash and checks to the Ag Center, or mail them to 900 Newnan Road, Carrollton, GA.
* Note: We must have registration AND payment for you to be fully registered. The first 25 fully registered will be included in the class.
